When medical and minimally invasive options fail for benign prostatic hyperplasia (BPH), surgical treatment options such as transurethral resection of the prostate (TURP) or open prostatectomy should be considered.
Open surgery for prostate removal is the procedure to remove the prostate gland to treat an enlarged prostate. Before the procedure, your doctor may want to perform a cystoscopy (the procedure of looking inside your urethra and bladder with a camera). You may need to stop taking certain medications, such as aspirin, kumadin (warfarin), Plavix (clopidogrel bisulfate), ibuprofen, or naproxen a few weeks before surgery.
Open prostatectomy is performed through a surgical incision in the lower abdomen.
The incision can go from the belly button to just above the pubic bone, or it can be made horizontally just above the pubic bone. The bladder is opened through this incision and the prostate gland is removed.
The surgeon removes only the inside of the prostate gland. The exterior is left behind. The process is similar to removing the core from an orange and leaving the peel intact. A drainage tube may be left in your abdomen after surgery. A urinary catheter is also placed in the bladder. These catheters allow the bladder to rest and heal.

AFTER SURGERY

After surgery, you will have drain tubes in your body to drain excess fluid. You will have a catheter in your bladder to empty your urine for a few days.
In most cases, patients stay at hospital for 2-7 days after prostatectomy. Your hospital staying time will depend on your medical situation and the type of procedure you undergo.
Have someone else drive you home. Do not drive until we tell you it is safe to drive.
Your urinary catheter will stay in place for 5 to 10 days.
You can return your normal activities after 4-6 weeks.
The surgery itself carries very few risks. As with any surgery, you are likely to get an infection or bleeding more than expected during or after surgery. These complications are rare and usually do not cause long-term health problems.
